PSM Gap Assessments: What We Review
Our gap assessment covers all fourteen elements of OSHA PSM (29 CFR 1910.119), with particular depth in the areas most commonly cited during OSHA inspections in Region 8:
- Process Safety Information (PSI). Are your P&IDs current? Does your PSI package reference current RAGAGEP editions? Is your equipment documentation complete and accessible?
- Process Hazard Analysis (PHA). Are your PHAs current and revalidated on schedule? Do they reference applicable engineering standards at the node level?
- Management of Change (MOC). Is your MOC process being consistently applied? Are temporary changes tracked? Are PSI documents updated following MOC completion?
- Mechanical Integrity (MI). Do your inspection frequencies align with current API inspection codes? Are inspection records complete and retrievable?
- Operating Procedures. Are your procedures written to current safe operating limits? Are they reviewed and certified annually as required?
